Is passive vaping harmful to non-vapers?
All published analyzes of the air in steamed rooms show that the concentrations of pollutants are far below the recognized limit values for air quality. This also applies to nicotine, which >95% remains in the body, so only traces of it are exhaled and released into the surrounding air. German authorities regularly claim that inhalation of propylene glycol, a substance also used to produce stage fog, irritates the respiratory tract. The European Chemicals Agency (ECHA) rejected the application from the Federal Institute for Occupational Safety and Health (BAuA) to classify propylene glycol as a respiratory irritant in December 2016 after thorough examination. There is no evidence of harmful effects from inhaling propylene glycol. Based on the available data, damage to health caused by passive vaping can be ruled out. However, it is a matter of politeness to be considerate of the well-being of other people and to vape discreetly in public or, if necessary, to refrain from vaping.

Do e-cigarettes and flavored liquids tempt young people to smoke?
Epidemiological studies indicate that increasing use of e-cigarettes by adolescents is associated with decreasing smoking rates. The idea that the use of e-cigarettes encourages minors to switch to tobacco cigarettes is implausible and is not supported by the available data. Flavorings enhance the taste and thus the attractiveness of e-cigarettes for adults. The alleged seduction of minors would at best be an argument for better monitoring of compliance with youth protection laws, but in no way justifies the restriction of the free availability of e-cigarettes for adults.

Is nicotine a highly potent neurotoxin and addictive toxin that damages the heart and blood vessels?
In normal doses (vaping, smoking), nicotine imitates part of the effects of a substance produced in the body (acetylcholine) and thereby activates nerve cells in the brain and the autonomic nervous system. The effect as a neurotoxin, i.e. blocking the function of nerve cells, only occurs in the event of a massive overdose, which is not achieved by inhalation. Highly potent neurotoxins are not available in pharmacies without a prescription. The addiction of smokers is due to a combined effect of nicotine with other components of tobacco smoke together with conditioned behavior, the “smoking ritual”. In the absence of tobacco smoke, the addictive potential of nicotine is very low, so that most vapers feel significantly less addictive pressure than smokers. In addition, smokers do not die because of their addiction but because of the harmful effects of the ingredients in tobacco smoke. In the cardiovascular system, nicotine, similar to caffeine, leads to a slight increase in blood pressure and heart rate. These effects are clinically harmless; the risk of serious illness (heart attack, stroke) or mortality is not increased by nicotine. However, people with severe cardiovascular diseases should avoid consuming nicotine.

Do particles in the vapor settle in the lungs and impair lung function?
In contrast to combustion smoke, the aerosol (mist) formed by e-cigarettes does not contain solid particles (tar), but rather liquid droplets that cannot settle in the lungs. The substances dissolved in the droplets are exhaled or absorbed by the epithelial cells (oral mucosa, respiratory tract). Clinical studies show that switching from smoking to vaping improves lung function in people with bronchial asthma or COPD and that years of vaping have no effect on lung function in non-smokers. The nature of the aerosol from e-cigarettes is physically identical to aerosols that are used for inhalation therapy of lung diseases. The smaller the liquid droplets in the aerosol, the deeper they penetrate into the lungs. Deep penetration into the lungs promotes the absorption of dissolved substances, but does not result in the droplets having any harmful effect on health. It is clinically irrelevant whether the droplets dissolve in the upper respiratory tract or in the bronchioles.

Does the vapor contain carcinogenic substances for which there is no lower limit?
When the liquid is heated, small amounts of aldehydes, especially formaldehyde, are formed. Formaldehyde is an endogenous metabolite that circulates in the blood and is exhaled by humans. During normal use, the vapor contains similar concentrations of formaldehyde as the ambient air. There are also frequent warnings about nitrosamines in steam. Nitrosamines are contained in almost all foods; these compounds can only be detected in trace amounts, if at all, in the aerosol of e-cigarettes. According to the basic pharmacological law of dose dependence of effects, there is always a concentration or dose of a substance that does not have a harmful effect. Otherwise, limit values for pollutants in the environment and food would be meaningless.

Can aromatic substances be harmful if inhaled?
In principle, counterfactual claims cannot be refuted. When inhaled, aromatic substances “could” improve lung function (or protect against hair loss, pimples, sweaty feet, etc.). There is no evidence that harmless food flavors turn into dangerous toxins when inhaled rather than consumed.

Are there long-term studies?
The apparent persuasiveness of this argument is based on the false association of vaping with smoking, the harmfulness of which was only recognized decades later. Vaping and smoking may seem similar, but they are not. When tobacco is burned, hundreds of carcinogenic or otherwise toxic substances are formed that are not produced when liquids evaporate. The reference to a lack of long-term studies is a false argument that can be used to prevent any new product. Decades of observational studies are not available when it comes to the approval of medicines or the market launch of new foods. If there is no evidence of harm and no plausible reasons to assume harm, the products should be classified as safe. And that is definitely the case with e-cigarettes.
